Consultation request form

When you submit a consultation request, the name, contact details and message you provide are sent to the team at Guildford Orthodontic Centre so they can contact you to arrange an appointment. The form is not intended for health records, images or radiographs; please do not include them. Submissions are delivered by email to office@surreyorthodontist.com, which the clinic's team monitors, and are stored by the site's hosting provider (Netlify) for the period needed to handle the request.

Analytics

The site uses analytics to understand which guides are read and how visitors move between pages. The site uses Google Analytics (via a Google tag) to measure page views and events such as form submissions and click-to-call. No advertising tags are used.

Cookies

The site sets only the cookies needed for analytics as described above. It does not use advertising cookies.

Your rights

You may ask what personal information the site or the clinic holds about you, request a correction, or withdraw a consultation request, by emailing office@surreyorthodontist.com.
